Group & Individual Pension Plans

| Authorises us to accept instructions submitted by an adviser on behalf of a client. For clients with plans within the Individual Pension Range (excluding EPPs). | |
| This form is used to provide further details of any transfer-in payment that is part of a block transfer to an individual or group pension plan with Scottish Life. | |
| Clients wishing to nominate who they would like to receive any pension benefits on their death. | |
| If a client is taking early retirement on the grounds of ill health. | |
| Money laundering form for an individual or a non-private individual (e.g. a third party). A separate form must be completed for each individual that is being verified. |
Life Policies
