Item What's it for?

Adviser Instructions Authorisation Form

Authorises us to accept instructions submitted by an adviser on behalf of a client. For clients with plans within the Individual Pension Range (excluding EPPs).

Block Transfer Form

This form is used to provide further details of any transfer-in payment that is part of a block transfer to an individual or group pension plan with Scottish Life.

Payment of Benefits on Death Form

Clients wishing to nominate who they would like to receive any pension benefits on their death.

Ill Health Form

If a client is taking early retirement on the grounds of ill health.

Customer Identity Verification Form

Money laundering form for an individual or a non-private individual (e.g. a third party). A separate form must be completed for each individual that is being verified.

Life Policies

Lost Policy Form

If a client loses their policy. A counterpart will be sent out as this is needed to declare the other policy as lost.

Deed of Assignment

This form is for use where a life policy is being assigned.

Direct Debit Mandate

If a client wishes to set up a direct debit to make payments.
