If your policy
- was in force on 11 September 2000 and still in force as at 1 July 2001, and
- was issued in your own name, and
- was not assigned to any other party prior to 1 July 2001
and you have not already received a payment of £500 for loss of membership rights you may be included on the list of former members who have still to claim their compensation payment.
